The East of England Cancer Alliances’ Primary Care Group (PCG) champions the needs of patients and carers in the development and delivery of the cancer transformation programme. The PCG acts as a link between primary care in the East of England and other parts of the health care system to drive transformational change in all relevant steps in the cancer pathway.
With clinicians and NHS colleagues across the East of England, the PCG have developed Two-Week Wait/Urgent Suspected Cancer (USC) referral proformas to support GP referrals to secondary care. The materials will soon be updated with the nomenclature change to USC as a result of the new Cancer Waiting Times update:
- Urgent Suspected Cancer Referral Form: Gynaecology
- Urgent Suspected Cancer Referral Form: Lower Gastrointestinal
- Urgent Suspected Cancer Referral Form: Prostate
- Urgent Suspected Cancer Referral Form: Upper Gastrointestinal
- Urgent Suspected Cancer Referral Form: Skin
Work to develop forms for other pathways is underway.
The generic information leaflet for health profesisonals to support cancer referrals is provided here. (entitled - 'Important information about your urgent suspected cancer referral').
